Membership & Volunteer Registration
DPUP offers two types of membership, depending on whether the applicant is a person with a disability or a person without a disability.
1. Membership for Persons with Disabilities
Persons with disabilities who wish to become a DPUP member should complete the Membership Form.
How to Apply
- Complete the membership form with accurate information.
- Sign the completed form or provide a thumb impression.
- Email the completed form to dpup.bangladesh@gmail.com .
- Attach the required supporting documents.
- Recent passport-size photograph
- Disability Identity Card/Certificate
- Birth Registration Certificate or other applicable supporting document
2. Associate Membership / Volunteer Registration
Persons without disabilities who wish to support DPUP’s activities can apply as an Associate Member or Volunteer using the same registration form.
How to Apply
- Complete the Associate Member/Volunteer form.
- Sign the completed form or provide a thumb impression.
- Email the completed form to dpup.bangladesh@gmail.com .
- Attach the required supporting documents.
- Recent passport-size photograph
- National Identity Card (NID) or Birth Registration Certificate
Important
All applicants are requested to ensure that the information and supporting documents provided are complete, accurate and valid. DPUP will review the application and communicate with the applicant regarding the next steps.
